Jackhammer Esophagus

Videos
Jackhammer esophagus is a hypercontractile esophageal motility disorder defined by extremely strong, high-pressure contractions during swallowing. Unlike diffuse esophageal spasm, contractions in jackhammer esophagus are coordinated but excessively forceful. These powerful contractions can cause severe chest pain, dysphagia, and regurgitation. Diagnosis requires high-resolution esophageal manometry.
Causes may include nerve dysfunction, esophageal inflammation, GERD, stress, anxiety, or idiopathic factors. Some cases are associated with early achalasia or other motility disorders. Temperature-triggered spasms may also contribute.
Risk factors include GERD, esophageal inflammation, stress, anxiety, autoimmune conditions, and prior foregut surgery. Cold or hot liquids may trigger symptoms.
Excessively strong contractions create intense chest pain, difficulty swallowing, and episodes of food sticking. These contractions can mimic cardiac pain and significantly impact eating and quality of life.
Complications include persistent dysphagia, weight loss, food impaction, esophageal dilation, and progression to achalasia in rare cases.
Symptoms include severe chest pain, difficulty swallowing, painful swallowing, regurgitation, and episodes of food sticking. Symptoms may be intermittent and triggered by temperature or stress.
Diagnosis is made through high-resolution esophageal manometry, which shows extremely high-pressure contractions. Additional testing may include endoscopy, barium swallow, and evaluation for GERD or anatomical defects.
Conditions that mimic jackhammer esophagus include cardiac chest pain, diffuse esophageal spasm, achalasia, EGJOO, GERD, and eosinophilic esophagitis.
Lifestyle strategies include avoiding trigger foods, managing stress, eating slowly, and avoiding very hot or cold liquids. Treating underlying reflux may reduce contraction intensity.
Treatment includes medications to relax esophageal muscle, management of reflux, botulinum toxin injection, dilation, and in severe cases, surgical or endoscopic therapy. Treatment is tailored to symptom severity and underlying causes.
Seek evaluation if you experience severe chest pain, difficulty swallowing, or episodes of food sticking, especially if symptoms mimic cardiac pain.
Treatment is recommended when symptoms interfere with eating, cause pain, or persist despite lifestyle changes.
Long-term outcomes depend on symptom severity and response to therapy. Regular follow-up helps monitor progression and manage underlying conditions.
Many patients improve with medical therapy and lifestyle modification. Severe or persistent cases may require procedural intervention.
Dr. Nestor de la Cruz-Muñoz provides advanced evaluation and treatment for esophageal motility disorders, offering expertise in both medical and procedural management of jackhammer esophagus.
